In this section we have included forms that need to be filled out before your first service at Helen's Haven. Click here to download the Welcome form. For your convenience you may download them and fill them out at home. We do ask that you arrive 10 minutes early for your first appointment. That will give us time to read through your forms and get to know you and your skin a little. Download the AHA/BHA Consent form. We have also included a copy of our salon policies. We do ask that you read and sign them. This will avoid any misunderstandings. If you would like to have before and after pictures taken to track your progress during treatments, we ask that you fill out the form to allow us to do that.
