For your convenience you may download them and fill them out at home. We do ask that you arrive 10 minutes early for your first appointment. That will give us time to read through your forms and get to know you and your skin a little.
We have also included a copy of our salon policies. We do ask that you read and sign them. This will avoid any misunderstandings.
